Enhanced Primary Care (EPC) (Chronic Disease Management) - GPs in the Medicare Benefits Schedule (MBS) allow physicians to schedule and co-ordinate health care for chronically or terminally ill people, and those with these illnesses, who need multi-disciplinary, team-oriented care from a family physician and at least two other health or care provider.

Long-term illness is an illness that has existed (or is likely to exist) for six month or more, such as Asthma, Krebs, Cardiovascular diseases, Diabetics, Musculoskeletal diseases and Strokes. While there is no shortlist of eligibility criteria, the CPM points are intended for those who need a structural response, as well as those who need continuous care from a multi-disciplinary group.

The eligibility of a client for Covered Disease Management (CDM) service is a clinician's verdict that takes into consideration the patient's state of health and care needs, as well as the general guidelines laid down in the MBS. Chronically ill individuals with complicated care needs who are administered by their GP under a GP Management Plan (item 721) and Team Care Arrangements (item 723) are entitled to Medicare discounts for certain related health care benefits upon referred by their GP.
